Based on the histological concept in which living tissues are formed by cells joined by thin elastic tissue and with nerve fibrils, capillaries, lymphatic and blood vessels. The disruption of these cells by surgical trauma provides the release of enzymes that delay healing. For this reason, surgical trauma should be minimized. Objective: Aimed to perform a brief review of the main considerations of virtual and minimally traumatic surgery in the context of maxillofacial surgery. Methods: The model used for the review was PRISMA. We used databases such as Scielo, Lilacs, Google Scholar, PubMed. The Cochrane Instrument was used to assess the risk of bias of the included studies. Results and Conclusion: A total of 118 articles were found involving virtual and minimally invasive surgery in maxillofacial procedures. A total of 56 articles were fully evaluated and 28 were included in this study. the main considerations regarding minimally traumatic surgery are the attempt to minimize the professional's effort, reduce surgical time and alleviate bleeding and inflammatory processes, edema, pain, and ecchymosis that can affect patients. However, the implants were long and had to be longer than 11 mm to be considered functional. Short implants compared to long ones require less remaining bone, reducing the patient's exposure to surgeries for bone grafting, the elevation of the maxillary sinus mucosa, and repositioning of the inferior alveolar nerve. Added to this, several dental treatments have benefited from this digital advance. Objective: To review the literature on short implants installed in both dental arches, evaluating their advantages, disadvantages, indications, and contraindications in the context of virtual surgery. Methods: The present study followed a systematic review model (PRISMA). The search strategy was performed in the PubMed, Scielo, Cochrane Library, Web of Science and Scopus, and Google Scholar databases. Results: A total of 110 articles were found, 64 articles were evaluated and 35 were rejected for not meeting the GRADE classification, and only 29 articles were used in this study to compose the textual part. Based on these findings, it was found that in remote years, authors reported that regions with reduced bone height are favored with the use of short implants not only because of their dimensions but also because of their surface treatment, which suggests that it is an important factor for achieving 100% success rates. The advantages of short implants are related to the simplicity of the technique, installation of implants in remaining bone, avoidance of bone grafts that present questionable results in the increases in height of the posterior alveolar ridge of the mandible, reduction of treatment time, and reduction of costs for the patient. A systematic review and meta-analysis studies analyzed the accuracy of implant placement using computer-guided surgery. A randomized study compared the precision of guided planning of new computer-assisted implant placement techniques, based on models that use CAD/CAM. Conclusion: It was concluded that short implants are a reliable, safe, and practical alternative to be used in any necessary location or situation. They do not show bone loss or resorption over the years, nor are they at risk of fracture or any damage to patients. They are safe to use, as long as they have an adequate design, therefore, fundamental tools in the dental clinic.
[[2022,5,9
["Major clinical findings of short implants and virtual surgery: a systematic review
[[2022,5,8
MedNEXT Journal of Medical and Health Sciences
[[2022,5,9
[[2022,5,8
[[2022,5,8
https://doi.org/10.54448/mdnt22s304
2763-5678
[[2022,5,8
2023,6,27
[[2023,6,25
["MedNEXT
Introduction: In the context of implant dentistry, what represents a great burden in the aesthetic recovery of patients who have suffered dentoalveolar trauma is the lack of bone in the alveolar crest, compromising the quality of life due to tooth loss. Several microRNAs participate in signaling networks through GM intervention. The interaction between GM and miRNAs plays a crucial role in vascular dysfunction. GM can metabolize L-carnitine, choline, and phosphatidylcholine and produce vascular-toxic metabolites such as trimethylamine-Noxide (TMAO), which is associated with the atherosclerotic process. Nutrology and dietary therapy represent important strategies, especially with the use of plant-derived miRNAs to modify GM. Objective: To carry out a systematic review to highlight the main roles of the gut microbiota and microRNAs in cardiovascular disease events. Methods: The present study followed a concise systematic review model (PRISMA). The literary search process was carried out from March to May 2023 and was developed based on Scopus, PubMed, Science Direct, Scielo, and Google Scholar, using scientific articles from 2002 to 2022. The low quality of evidence was attributed to reports of cases, editorials, and brief communications, according to the GRADE instrument. The risk of bias was analyzed according to the Cochrane instrument. Results and Conclusion: The total of 126 studies were found for eligibility analysis, and then 42 of the 64 total studies were selected for this systematic review. According to the GRADE instrument, most studies showed homogeneity in their results, with X2 =88.7%>50%. It was concluded that the gut microbiota can be influenced by diet, genetics, and environmental factors. Changes in the microbial population can lead to multiple diseases, including cardiovascular disease. miRNAs have been known as important regulators of several biological functions, and are also involved in the pathogenesis of cardiovascular diseases. There is evidence that disturbances in the gut microbiota and derived metabolites impair the development of atherosclerotic cardiovascular disease. The records of the first surgeries for the correction of dentofacial deformities date from the mid-nineteenth century and were initially limited to mandibular surgeries. The malocclusion has as one of the primary etiological factors the facial growth pattern. Angle's statement already said that the only possibility of correcting true dentofacial deformities was the combination of orthodontics with surgery, and the importance of the combined effort of these two distinct areas in the approach to dentofacial disharmony in patients with problems was recognized skeletal. Objective: This study aimed to review the literature on OS involving mandible, maxilla, mento and associations. Methods: Clinical studies with qualitative and/or quantitative analysis were included, following the rules of the systematic review-PRISMA. Results: A total of 107 articles were found involving “orthognathic surgery". A total of 47 articles were evaluated in full, and 33 were included and discussed in this study. Individuals with class III dentofacial deformities are those that normally present greater aesthetic and functional impact, and for this reason, are the ones that more frequently seek treatment. However, some studies have shown a tendency for complications to occur in older patients submitted to orthognathic surgeries. Mandibular surgical procedures with maxillary segmentation and combining three types of osteotomies should be carefully planned and trained to reduce the occurrence of complications. The surgeon, orthodontist, and all staff involved should be focused on avoiding complications during all phases of treatment. Conclusion: It was concluded that there was an increase in the cases of OS in the last years, and with homogeneous samples between the masculine and feminine genres, and the advances in maxillary surgery corresponded to the greater number of surgical treatments.
[[2022,4,5
["Ortho-surgical In the setting of bucomaxillo facial surgery, bone tissue is a specialized connective tissue made up of cells and a mineralized extracellular matrix. To reduce the excessive resorption observed in these pathologies, it has been studied more rigorously in pre-clinical and clinical studies to improve anti-resorptive drugs that allow for treating or preventing pathologies of bone metabolism, such as bisphosphonates (BP) and denosumab (DN) as well as some angiogenesis inhibitors, can induce osteonecrosis of the mandible (ONJ). Methods: The present study was followed by a systematic review model (PRISMA). The search strategy was performed in the PubMed, Cochrane Library, Web of Science and Scopus, and Google Scholar databases. The Cochrane Instrument was used to assess the risk of bias from the included studies. The present study was carried out from December 2022 to January of 2023. Results and Conclusion: A total of 108 articles were found. Initially, duplicate articles were excluded. After this process, the abstracts were evaluated and a new exclusion was performed based on the GRADE Instrument and Risk of Bias. A total of 55 articles were fully evaluated and 22 were included and discussed in this study. The American Association of Oral and Maxillofacial Surgeons proposed for the first time its nomenclature "Bisphosphonates Related Osteonecrosis of the Jaws". ONJ is the term used to describe bone cell death when the osteocyte becomes necrotic. Osteonecrosis of the jaws can be considered a severe adverse effect of BP therapy. Osteonecrosis of the jaws induced by the use of antiresorptive drugs commonly occurs in the oral cavity, mainly because the bone tissue is covered and protected only by a thin layer of periosteum and epithelium. Bisphosphonates inhibit RANKL expression, as well as stimulate OPG production by bone marrow cells and osteoblasts, inhibiting the RANK-RANKL interaction. These synergistic actions lead to a decrease in the recruitment of osteoclasts and, consequently, to a reduction in bone resorption.
[[2023,4,10
["Relationship of the occurrence and main approaches to osteonecrosis of the jaw with the use of bisphosphonates: a concise systematic review
[[2023,4,4
MedNEXT Journal of Medical and Health Sciences
[[2023,4,10
[[2023,4,4
[[2023,4,4
https://doi.org/10.54448/mdnt23s201
2763-5678
[[2023,4,4
2025,8,2
[[2025,7,30
["IJN
Introduction: In the context of obesity and its comorbidities, more than 2.2 billion people are overweight or obese worldwide. By 2030, it is estimated that more than 60% of the world's population will be overweight or obese. Brazil has approximately 20 million obese people. In this context, and as a measure to mitigate this pathological scenario, nutrology combined with the use of the intragastric balloon (IGB) is an important pillar for obesity treatment. Also noteworthy is the 1-year adjustable balloon option, the Spatz3®, which offers the advantage of longer treatment with the possibility of adjustments to increase weight loss. Objective: the present study analyzed, through a systematic review, the main information and results of the use of the Spatz3® intragastric balloon, highlighting the volume adjustments up and down. Methods: The model followed for the systematic review was PRISMA. Databases such as Scopus, Embase, Cochrane, Scielo, Lilacs, Google Scholar, PubMed were used. A total of 136 clinical studies were submitted to the eligibility analysis and, after that, 20 studies were selected to compose the present study. The Risk of Bias was analyzed according to the Cochrane Instrument model. Main findings and Conclusion: Studies have shown that the treatment of obesity and super obesity with Spatz3® is a safe and effective procedure for weight reduction, without mortality, but with greater morbidity compared to traditional BIGs. Studies have also shown that adjusting the Spatz3® volume upwards allowed the balloon to remain in place for a longer time. However, the effectiveness of the upward adjustment still requires further confirmation. MicroRNAs stand out, which are a class of small non-coding RNAs that regulate gene expression. Changes in their expression and functions have been associated with several diseases, including metabolic disorders and obesity. Enteral and parenteral nutrition therapy functions as an important regulator of microRNAs against inflammatory and metabolic processes. Objective: It was to carry out a systematic review of the main approaches to enteral and parenteral nutrition therapy in patients with obesity, to regulate the gene expression of microRNAs to mitigate inflammatory processes and metabolic disorders. Methods: The PRISMA Platform systematic review rules were followed. The search was carried out from August to September 2024 in the Scopus, PubMed, Science Direct, Scielo, and Google Scholar databases. The quality of the studies was based on the GRADE instrument and the risk of bias was analyzed according to the Cochrane instrument. Results and Conclusion: A total of 117 articles were found. In recent years, the use of barbed tensor threads has gained popularity as a means of rejuvenating the face. Objective: This study presented the main clinical and aesthetic approaches to optimizing techniques using PDO threads for facial rejuvenation. Methods: The systematic review rules of the PRISMA Platform were followed. The search was carried out from December 2024 to January 2025 in the Scopus, PubMed, Science Direct, Scielo, and Google Scholar databases. The quality of the studies was based on the GRADE instrument and the risk of bias was analyzed according to the Cochrane instrument. Results and Conclusion: A total of 101 articles were found. A total of 17 articles were evaluated and 14 were included in this systematic review. Considering the Cochrane risk of bias tool, the overall assessment resulted in 38 studies with a high risk of bias and 35 studies that did not meet GRADE. Most studies showed homogeneity in their results, with X2=65.7%>50%. It was concluded that the combination of lower blepharoplasty and midface lift using PDO threads is a comprehensive and effective approach for facial rejuvenation. It significantly improves wrinkle reduction, midface lift, and patient satisfaction. Ultrasound-guided thread lift, a method of evaluating and performing midface lift, proves to be safe and efficient. This approach holds promise as a future option in anti-aging cosmetic surgery, presenting a minimally invasive alternative with natural-looking results and reduced downtime. The Cartesian Technique™ with Smart PDOs™ is an effective protocol for skin resurfacing that enhances cellular biostimulation through the release of bioactive drugs. Using PDO threads for midface lifting is simpler, faster, and less invasive than conventional surgical methods, and this method simultaneously achieves satisfactory results. The combination of hyaluronic acid fillers and PDO threads was effective in achieving and maintaining long-term improvements in facial volume and contour.
