We examined <jats:italic>TEL\/AML1<\/jats:italic> using nested reverse\u2010transcription polymerase chain reaction (RT\u2010PCR) and correlated <jats:italic>TEL\/AML1<\/jats:italic> with cytogenetics and immunophenotypes in 75 consecutively analyzed Chinese children with ALL in Hong Kong. <jats:italic>TEL\/AML1<\/jats:italic> was detected in 17.9% (12\/67) B\u2010lineage ALL at diagnosis but not in 8 T\u2010ALL children or in 34 adults with ALL. <jats:italic>E2A\/PBX1, MLL\/AF4,<\/jats:italic> and <jats:italic>BCR\/ABL<\/jats:italic> were not found in <jats:italic>TEL\/AML1<\/jats:italic>+ patients. Coexpression of cross\u2010lineage antigens was associated with <jats:italic>TEL\/AML1<\/jats:italic> gene fusion (<jats:italic>p<\/jats:italic> = 0.032), with CD13 in 80% (4\/5) <jats:italic>TEL\/AML1<\/jats:italic>+ cohort. Chromosomal abnormalities were demonstrated in 50% of the <jats:italic>TEL\/AML1<\/jats:italic>+ ALL; however, a cryptic t(12;21) was not detected in these cases. Hyperdiploidy of 47\u201348 chromosomes was encountered in 25%. Deletion of 12p resulting in the loss of the normal allele of <jats:italic>TEL<\/jats:italic> and nonspecific del(6q) were noted in 8% (1\/12) and 25% (3\/12) of the <jats:italic>TEL\/AML1<\/jats:italic>+ children, respectively. Rapid clearance of <jats:italic>TEL\/AML1<\/jats:italic> was noted in 50% of the patients on completion of the induction therapy; however, 16.7% (2\/12) <jats:italic>TEL\/AML1<\/jats:italic>+ ALL relapsed at a mean of 48.6 months from diagnosis (25 months off\u2010therapy). The incidence of relapses of <jats:italic>TEL\/AML1<\/jats:italic>+ ALL was comparable to that at diagnosis in B\u2010lineage ALL (14.3% [2\/14] vs. 17.9% [12\/67], <jats:italic>p<\/jats:italic> &gt; 0.05). The relapse rate in <jats:italic>TEL\/AML1<\/jats:italic>+ ALL was similar to that of <jats:italic>TEL\/AML1<\/jats:italic>\u2212 ALL (16.7% [2\/12] vs. 20.6% [13\/63], <jats:italic>p<\/jats:italic> &gt; 0.05). The duration of first complete remission in <jats:italic>TEL\/AML1<\/jats:italic>+ ALL was significantly longer as compared to <jats:italic>TEL\/AML1<\/jats:italic>\u2212 ALL (mean [range] in month: 48.6 [47.2 \u2212 50] vs 14.6 [2.9 \u2212 42.3], <jats:italic>p<\/jats:italic> &lt; 0.0001). Irrespective of <jats:italic>TEL\/AML1<\/jats:italic> rearrangement, the probabilities of the five\u2010year overall survival and the event\u2010free survival of patients were comparable (overall survival: 100% vs. 72.3%, <jats:italic>p<\/jats:italic> = 0.166 and event\u2010free survival: 60% vs. 56.2%, <jats:italic>p<\/jats:italic> = 0.343).
