The results answer some natural questions which have arisen in preparing a new edition of <jats:ext-link xmlns:xlink=\"http:\/\/www.w3.org\/1999\/xlink\" xlink:href=\"#bib5\">5<\/jats:ext-link>. To make the paper self\u2010contained we define all of the cardinal functions that enter into the theorems here.
