{"status":"ok","message-type":"work","message-version":"1.0.0","message":{"indexed":{"date-parts":[[2026,3,12]],"date-time":"2026-03-12T16:17:53Z","timestamp":1773332273226,"version":"3.50.1"},"reference-count":42,"publisher":"Springer Science and Business Media LLC","issue":"4","license":[{"start":{"date-parts":[[2022,12,20]],"date-time":"2022-12-20T00:00:00Z","timestamp":1671494400000},"content-version":"tdm","delay-in-days":0,"URL":"https:\/\/creativecommons.org\/licenses\/by\/4.0"},{"start":{"date-parts":[[2022,12,20]],"date-time":"2022-12-20T00:00:00Z","timestamp":1671494400000},"content-version":"vor","delay-in-days":0,"URL":"https:\/\/creativecommons.org\/licenses\/by\/4.0"}],"content-domain":{"domain":["link.springer.com"],"crossmark-restriction":false},"short-container-title":["Diabetologia"],"published-print":{"date-parts":[[2023,4]]},"abstract":"<jats:title>Abstract<\/jats:title><jats:sec>\n                <jats:title>Aims\/hypothesis<\/jats:title>\n                <jats:p>The aims of this study were to assess cognitions relating to hypoglycaemia in adults with type 1 diabetes and impaired awareness of hypoglycaemia before and after the multimodal HypoCOMPaSS intervention, and to determine cognitive predictors of incomplete response (one or more severe hypoglycaemic episodes over 24 months).<\/jats:p>\n              <\/jats:sec><jats:sec>\n                <jats:title>Methods<\/jats:title>\n                <jats:p>This analysis included 91 adults with type 1 diabetes and impaired awareness of hypoglycaemia who completed the Attitudes to Awareness of Hypoglycaemia (A2A) questionnaire before, 24 weeks and 24 months after the intervention, which comprised a short psycho-educational programme with optimisation of insulin therapy and glucose monitoring.<\/jats:p>\n              <\/jats:sec><jats:sec>\n                <jats:title>Results<\/jats:title>\n                <jats:p>The age and diabetes duration of the participants were 48\u00b112 and 29\u00b112 years, respectively (mean\u00b1SD). At baseline, 91% reported one or more severe hypoglycaemic episodes over the preceding 12 months; this decreased to &lt;20% at 24 weeks and after 24 months (<jats:italic>p<\/jats:italic>=0.001). The attitudinal barrier \u2018hyperglycaemia avoidance prioritised\u2019 (\u03b7<jats:sup>2<\/jats:sup><jats:sub>p<\/jats:sub>=0.250, <jats:italic>p<\/jats:italic>=0.001) decreased from baseline to 24 weeks, and this decrease was maintained at 24 months (mean\u00b1SD=5.3\u00b10.3 vs 4.3\u00b10.3 vs 4.0\u00b10.3). The decrease in \u2018asymptomatic hypoglycaemia normalised\u2019 from baseline (\u03b7<jats:sup>2<\/jats:sup><jats:sub>p<\/jats:sub>=0.113, <jats:italic>p<\/jats:italic>=0.045) was significant at 24 weeks (1.5\u00b10.3 vs 0.8\u00b10.2). Predictors of incomplete hypoglycaemia response (one or more further episodes of severe hypoglycaemia) were higher baseline rates of severe hypoglycaemia, higher baseline scores for \u2018asymptomatic hypoglycaemia normalised\u2019, reduced change in \u2018asymptomatic hypoglycaemia normalised\u2019 scores at 24 weeks, and lower baseline \u2018hypoglycaemia concern minimised\u2019 scores (all <jats:italic>p<\/jats:italic>&lt;0.05).<\/jats:p>\n              <\/jats:sec><jats:sec>\n                <jats:title>Conclusions\/interpretation<\/jats:title>\n                <jats:p>Participation in the HypoCOMPaSS RCT was associated with improvements in hypoglycaemia-associated cognitions, with \u2018hyperglycaemia avoidance prioritised\u2019 most prevalent. Incomplete prevention of subsequent severe hypoglycaemia episodes was associated with persistence of the cognition \u2018asymptomatic hypoglycaemia normalised\u2019.
