Identifying preoperative risk factors for this complication can help patients\u2019 risk stratification and optimization. We performed a systematic review and meta-analysis to find predictors for early postoperative bleeding after Roux-en-Y gastric bypass (RYGB).<\/jats:p>\n              <\/jats:sec><jats:sec>\n                <jats:title>Methods<\/jats:title>\n                <jats:p>We conducted a systematic review, searching PubMed, Cochrane Library, and Web of Science until November 2023. We performed a random-effects meta-analysis to explore preoperative risk factors associated with early postoperative bleeding after RYGB. Sources of heterogeneity were explored by leave-one-out analyses.<\/jats:p>\n              <\/jats:sec><jats:sec>\n                <jats:title>Results<\/jats:title>\n                <jats:p>23 studies were included, comprising 232,488 patients. Male gender (meta-analytical RR\u2009=\u20091.42, 95%CI\u2009=\u20091.21\u20131.66, I<jats:sup>2<\/jats:sup>\u2009=\u200918%, Q Cochran test p-value\u2009=\u20090.29) and revisional surgery (meta-analytical RR\u2009=\u20091.35, 95%CI\u2009=\u20091.12\u20131.62, I<jats:sup>2<\/jats:sup>\u2009=\u200922%, Q Cochran test <jats:italic>p<\/jats:italic>\u2009=\u20090.21) were associated with higher risk of EPB. On average, patients with EPB were older than the remainder (MD for the mean age\u2009=\u20092.82 years, 95%CI\u2009=\u20090.97\u20134.67, I<jats:sup>2<\/jats:sup>\u2009=\u20090.00%, Q Cochran test <jats:italic>p<\/jats:italic>\u2009=\u20090.46). Except for hypertension (meta-analytical RR\u2009=\u20091.33, 95%CI\u2009=\u20091.02\u20131.73, I<jats:sup>2<\/jats:sup>\u2009=\u200966%, Q Cochran test <jats:italic>p<\/jats:italic>\u2009&lt;\u20090.0001), comorbidities were not associated with a higher risk of EPB.<\/jats:p>\n              <\/jats:sec><jats:sec>\n                <jats:title>Conclusion<\/jats:title>\n                <jats:p>Preoperative risk factors, including age, gender, hypertension, and revisional bariatric surgery, are associated with early postoperative bleeding after RYGB.
