The first description considered this protozoan a rare and harmless commensal, since then it has struggled to gain recognition as a pathogen. Commercial multiplex real-time PCR was used to detect <jats:italic>D. fragilis<\/jats:italic> in fecal samples from hospitalized children (\u2a7d18 years) with acute gastrointestinal disease, admitted to two hospitals of Lisbon area, with different demographic characteristics. A total of 176 children were studied, 103 (58\u00b75%) male, 144 (81\u00b78%) children between 0 and 5 years and 32 (18\u00b72%) above 6 years old. The overall protozoa frequency considering the four tested microorganisms were 8\u00b75% (15\/176), and the most frequently found protozoan was <jats:italic>D. fragilis<\/jats:italic>, 6\u00b73% (11\/176). <jats:italic>Dientamoeba fragilis<\/jats:italic> frequency was higher among older children (21\u00b79%), than younger children (2\u00b78%), and greater in boys (6\u00b78%) than in girls (5\u00b75%). All positive children presented with diarrhoea associated with vomiting, fever and abdominal pain. Infection was associated with the age of children (<jats:italic>P<\/jats:italic> &lt; 0\u00b7001), school attendance (<jats:italic>P<\/jats:italic> = 0\u00b7002) and consumption of certain foods (<jats:italic>P<\/jats:italic> = 0\u00b7014), e.g. cakes with cr\u00e8me and ham.
