The aim of this study was to characterize the disposition kinetics of 3\u2010HC in healthy smokers, including metabolism to (3\u2032R,5\u2032S)\u2010<jats:italic>trans<\/jats:italic>\u20103\u2032\u2010hydroxycotinine glucuronide (3\u2010HC\u2010Gluc). We also studied pharmacologic responses to 3\u2010HC and plasma levels of 3\u2010HC in a group of smokers.<\/jats:p><jats:p> <jats:bold>\n            <jats:italic>Methods\u2002<\/jats:italic>\n          <\/jats:bold>Eight cigarette smokers were studied on a clinical research ward. After 5\u2003days of supervised nonsmoking, each subject received an intravenous infusion of 3\u2010HC, 4\u2003\u00b5g\u2003kg<jats:sup>\u22121<\/jats:sup>\u2003min<jats:sup>\u22121<\/jats:sup> for 60\u2003min. Plasma and urine levels of 3\u2010HC and 3\u2010HC\u2010Gluc and cardiovascular and subjective responses were examined. Plasma levels of 3\u2010HC, nicotine, and cotinine were measured in 62 smokers on up to three occasions.<\/jats:p><jats:p> <jats:bold>\n            <jats:italic>Results\u2002<\/jats:italic>\n          <\/jats:bold>The total plasma clearance of 3\u2010HC averaged 1.3\u2003ml\u2003min<jats:sup>\u22121<\/jats:sup>\u2003kg<jats:sup>\u22121<\/jats:sup>, of which 63% was renal excretion of unchanged drug. An average of 29% of the dose was excreted as 3\u2010HC\u2010Gluc. 3\u2010HC did not have nicotine\u2010like cardiovascular effects.<\/jats:p><jats:p> <jats:bold>\n            <jats:italic>Conclusions\u2002<\/jats:italic>\n          <\/jats:bold>These findings extend our understanding of the quantitative nature of nicotine metabolism.
