To evaluate the impact of maternal lipid changes upon the fetus in pre\u2010eclampsia (PE) by evaluating lipid profile simultaneously in maternal and umbilical cord blood (UCB). <jats:italic>Design<\/jats:italic>. Case\u2010control study performed on healthy and pre\u2010eclamptic pregnant women and their neonates. <jats:italic>Setting<\/jats:italic>. The Department of Obstetrics and Gynecology, Hospital S. Joao and Faculty of Pharmacy, Porto, Portugal. <jats:italic>Samples<\/jats:italic>. Forty\u2010two healthy pregnancies and 46 pregnancies complicated with PE. <jats:italic>Methods<\/jats:italic>. Total cholesterol (TChol), HDL\u2010cholesterol (HDLc), LDL\u2010cholesterol (LDLc) and triglycerides (TG) levels were determined using enzymatic methods. Apolipoprotein (apo) A\u2010I, apoB and lipoprotein (a) [Lp(a)] values were measured by immunoturbidimetry. <jats:italic>Main outcome measures<\/jats:italic>. Fetal and maternal plasma levels of TChol, HDLc, LDLc, TG, apoA\u2010I, apoB and Lp(a). <jats:italic>Results<\/jats:italic>. Pre\u2010eclamptic women presented significantly higher values for TChol, LDLc, HDLc, TG, apoA\u2010I and apoB compared to normal pregnant women. In the UCB from pre\u2010eclamptic pregnancies, we observed significantly lower values for HDLc and apoA\u2010I, and significantly higher TG concentrations and LDLc\/HDLc ratio when compared to normal cases. A positive correlation was observed between maternal TG levels and proteinuria, a marker of PE severity (<jats:italic>r<\/jats:italic> =0.40, <jats:italic>p<\/jats:italic> &lt;0.01). <jats:italic>Conclusions<\/jats:italic>. Our data suggest that pre\u2010eclamptic pregnancy is associated with an enhanced hyperlipidemia, which seems to have a negative impact on fetal lipid profile, as reflected by a higher atherogenic LDLc\/HDLc ratio and higher TG levels.
