The 2024 KDIGO guidelines recommend identifying frailty as a key factor in CKD management. The\u00a0Comprehensive Geriatric Assessment (CGA)\u00a0is the gold-standard methodology used by geriatricians to define frailty. Our aim was to identify predictors of frailty in CKD patients, using CGA methodology.<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec>\n                    <jats:title>Method<\/jats:title>\n                    <jats:p>We evaluated\u00a0204 patients aged \u226565 years\u00a0in 2024:\u00a0111 with CKD stages 3 to 5,\u00a071 on dialysis, and\u00a022 receiving conservative care\u00a0in a single nephrology unit. A multidisciplinary team, including a nephrologist specialized in geriatrics, conducted the CGA. Data collection included\u00a0demographics and clinical parameters, as well as assessments of\u00a0activities of daily living (ADL), cognitive function, nutritional status, mobility, and social support. Statistical analysis was performed using SPSS v.29.<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec>\n                    <jats:title>Results<\/jats:title>\n                    <jats:p>Among the 204 patients, the\u00a0mean age was 79.1 (\u00b17.2) years,\u00a058.2% were male,\u00a032.7% were widowed, and\u00a028% were institutionalized,\u00a052.5% had diabetes, and\u00a027.7% had heart failure. Overall,\u00a025% of patients were classified as frail\u00a0based on the CGA. Frailty prevalence varied depending on the assessment tool used:\u00a026.5% according to the Fried Index (FI &amp;gt;3),\u00a023.5% according to the Edmonton Frailty Scale (EFS &amp;gt;8), and\u00a058.3% according to the Clinical Frailty Scale (CFS &amp;gt;5). Frailty prevalence also differed across CKD stages (Table\u00a01).<\/jats:p>\n                    <jats:p>Logistic regression identified four significant predictors of frailty: activities of daily livings [(Barthel Index (Exp(B) = 0.191, P\u00a0&amp;lt;\u00a00.001), Lawton and Brody Scale (Exp(B) = 0.145, P\u00a0=\u00a00.001)], mobility [Short Physical Performance Battery (SPPB) (Exp(B) = 0.575, P\u00a0=\u00a00.001)] and depression (Exp(B) = 21.057, P\u00a0=\u00a00.013). The model showed an excellent fit (Nagelkerke's R\u00b2 = 0.835, 92.3% accuracy).<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec>\n                    <jats:title>Conclusion<\/jats:title>\n                    <jats:p>Frailty identification has a significant impact on CKD management. Simple, time-efficient tools should be integrated into routine care to facilitate early detection and intervention. The\u00a0EFS\u00a0may be particularly useful due to its shorter administration time while maintaining alignment with CGA results. Identifying\u00a0frailty syndromes such as impaired mobility (SPPB) and depression\u00a0could help target modifiable risk factors and improve patient outcomes.
