Several clinical and histopathological markers have been proposed as potential predictors of long-term outcomes. Understanding how these factors impact disease prognosis may facilitate the implementation of personalized treatment strategies. This study aims to identify predictors of adverse outcomes in PMN.<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec>\n                    <jats:title>Method<\/jats:title>\n                    <jats:p>We conducted a retrospective analysis of 35 patients diagnosed with PMN between 2007 and 2022 at a tertiary center. Clinical and biochemical characteristics, along with kidney biopsy results, were assessed at baseline. Patients were divided into favorable and unfavorable outcome groups based on estimated glomerular filtration rate (eGFR), proteinuria, progression to end-stage renal disease or death at two years of follow-up. In addition to this classification, patients were categorized according to proteinuria levels higher or lower than 1 g\/g at two years of follow-up.<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec>\n                    <jats:title>Results<\/jats:title>\n                    <jats:p>Patients with adverse disease progression experienced significantly more thromboembolic events (29% vs. 0%, P\u00a0=\u00a00.019), and had a higher frequency of PLA2R positivity (90% vs. 42%, P\u00a0=\u00a00.033) at diagnosis. Univariate analysis revealed that both higher creatinine levels (OR = 3.854, 95% CI: 1.039\u201314.303, P\u00a0=\u00a00.044) and PLA2R positivity (OR = 12.0, 95% CI: 1.178\u2013122.274, P\u00a0=\u00a00.036) at diagnosis were significantly associated with unfavorable disease progression. In the multivariate logistic regression model, only PLA2R positivity remained significantly associated with adverse outcomes (OR = 11.31, 95% CI: 1.039\u2013123.286, P\u00a0=\u00a00.016). Analysis of proteinuria at two years revealed that PLA2R levels at diagnosis were significantly higher in patients with proteinuria &amp;gt;1 g\/g (U = 28, P\u00a0=\u00a00.038), and were associated with proteinuria &amp;gt;1 g\/g at two years in the univariate analysis (OR = 1.010, 95% CI: 1.000\u20131.019, P\u00a0=\u00a00.040). Notably, histopathological characteristics had no impact on the results at two years.<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec>\n                    <jats:title>Conclusion<\/jats:title>\n                    <jats:p>PLA2R positivity is a predictor of disease progression, with higher PLA2R levels correlating with the development of significant proteinuria at two years of follow-up. This finding suggests that immune-mediated mechanisms, particularly those involving PLA2R, modulate the clinical response, potentially influencing disease course and therapeutic outcomes.
