{"status":"ok","message-type":"work","message-version":"1.0.0","message":{"indexed":{"date-parts":[[2026,3,16]],"date-time":"2026-03-16T15:44:00Z","timestamp":1773675840666,"version":"3.50.1"},"reference-count":17,"publisher":"Ovid Technologies (Wolters Kluwer Health)","issue":"3","content-domain":{"domain":[],"crossmark-restriction":false},"short-container-title":[],"published-print":{"date-parts":[[2018,3]]},"abstract":"<jats:sec>\n            <jats:title>Background<\/jats:title>\n            <jats:p>Even though mass gastric cancer screening is controversial in Western countries, diverse strategies have been proposed in intermediate-risk to high-risk areas.<\/jats:p>\n          <\/jats:sec>\n          <jats:sec>\n            <jats:title>Aim<\/jats:title>\n            <jats:p>The aim of this study was to determine the long-term accuracy of the pepsinogen test (PG) for early diagnosis of gastric cancer.<\/jats:p>\n          <\/jats:sec>\n          <jats:sec>\n            <jats:title>Participants and methods<\/jats:title>\n            <jats:p>A cohort of inhabitants from the Northern part of Portugal, aged between 40 and 74 years (<jats:italic toggle=\"yes\">n<\/jats:italic>=5913), subjected to the PG test (PGI\u226470\u2009ng\/ml and PGI\/PGII\u22643), were followed up between November 2006 and December 2015. The diagnosis of gastric cancer was determined through linkage to the population-based registry of cancer [North Region Cancer Registry of Portugal (RORENO)].<\/jats:p>\n          <\/jats:sec>\n          <jats:sec>\n            <jats:title>Results<\/jats:title>\n            <jats:p>Twenty-six gastric cancers were diagnosed (0.4%): nine (4%) among individuals \u2018positive\u2019 for the PG test (<jats:italic toggle=\"yes\">n<\/jats:italic>=225) and 15 (0.3%) among those who were \u2018negative\u2019 (<jats:italic toggle=\"yes\">n<\/jats:italic>=5688) [hazard ratio=12.7; 95% confidence interval (CI): 5.6\u201328.6]. Individuals with a \u2018negative PG test\u2019 had a 3-year risk of gastric cancer of 0.1%, representing a sensitivity of 35% (95% CI: 17\u201356%), globally, and of 58% (95% CI: 28\u201385%) at 3 years follow-up. The median survival rate in both groups was over 24 months.<\/jats:p>\n          <\/jats:sec>\n          <jats:sec>\n            <jats:title>Conclusion<\/jats:title>\n            <jats:p>The PG test was found to be suboptimal as a screening test and, if used (before upper gastrointestinal endoscopy), it is mandatory to repeat it after 3 years.<\/jats:p>\n
