This paper proposes an adaptive region\u2010aware image compression framework for healthcare imaging over O\u2010RAN that reduces fronthaul load while preserving diagnostically relevant information. Each image is partitioned into Region\u2010of\u2010Interest (ROI) and Non\u2010ROI areas and compressed using independent quantisation parameters. An optimisation model is formulated to minimise transmitted data size subject to ROI and Non\u2010ROI quality constraints, end\u2010to\u2010end latency bounds and computational limits at O\u2010RAN nodes. The framework is evaluated using two medical imaging datasets (chest X\u2010rays and bone fracture X\u2010rays), where empirical rate\u2013distortion and quality models are derived and validated. Results demonstrate substantial fronthaul bandwidth reduction\u2014achieving compression ratios up to 416:1\u2014while maintaining ROI quality and diagnostic accuracy above 97%.
