{"status":"ok","message-type":"work","message-version":"1.0.0","message":{"indexed":{"date-parts":[[2026,3,6]],"date-time":"2026-03-06T19:35:05Z","timestamp":1772825705370,"version":"3.50.1"},"reference-count":35,"publisher":"Wiley","issue":"21-22","license":[{"start":{"date-parts":[[2015,8,27]],"date-time":"2015-08-27T00:00:00Z","timestamp":1440633600000},"content-version":"vor","delay-in-days":0,"URL":"http:\/\/onlinelibrary.wiley.com\/termsAndConditions#vor"}],"funder":[{"DOI":"10.13039\/501100001871","name":"Funda\u00e7\u00e3o para a Ci\u00eancia e a Tecnologia","doi-asserted-by":"publisher","id":[{"id":"10.13039\/501100001871","id-type":"DOI","asserted-by":"publisher"}]},{"DOI":"10.13039\/501100002924","name":"Federaci\u00f3n Espa\u00f1ola de Enfermedades Raras","doi-asserted-by":"publisher","id":[{"id":"10.13039\/501100002924","id-type":"DOI","asserted-by":"publisher"}]}],"content-domain":{"domain":[],"crossmark-restriction":false},"short-container-title":["Journal of Clinical Nursing"],"published-print":{"date-parts":[[2015,11]]},"abstract":"<jats:sec><jats:title>Aims and objectives<\/jats:title><jats:p>To analyse the <jats:styled-content style=\"fixed-case\">B<\/jats:styled-content>raden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale scores and sub\u2010scores assessed in <jats:styled-content style=\"fixed-case\">P<\/jats:styled-content>ortuguese hospitalised adult patients in association with their characteristics, diagnoses and length of stay.<\/jats:p><\/jats:sec><jats:sec><jats:title>Background<\/jats:title><jats:p>The <jats:styled-content style=\"fixed-case\">B<\/jats:styled-content>raden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale is used worldwide for pressure ulcer risk assessment and supports nurses in the implementation of preventive interventions.<\/jats:p><\/jats:sec><jats:sec><jats:title>Design<\/jats:title><jats:p>Retrospective cohort analysis of electronic health record database from adult patients admitted to medical and surgical areas during 2012.<\/jats:p><\/jats:sec><jats:sec><jats:title>Methods<\/jats:title><jats:p>Braden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale scores and sub\u2010scores of 8147 patients were associated with age, gender, type of admission (emergency service or programmed), specialty units (medical or surgical), length of stay, patient discharge (discharge, decease or transference to other hospital) and <jats:styled-content style=\"fixed-case\">ICD<\/jats:styled-content>\u20109 diagnosis.<\/jats:p><\/jats:sec><jats:sec><jats:title>Results<\/jats:title><jats:p>The participants with significantly lower <jats:styled-content style=\"fixed-case\">B<\/jats:styled-content>raden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale scores were women, older people, hospitalised in medical units, with emergency service admission, longer hospitalisation stays and\/or with vascular, traumatisms, respiratory, infection or cardiac diseases. Mobility, friction\/shear forces and activity had higher contributions to the Braden Scale score, while nutrition had the lowest contribution.<\/jats:p><\/jats:sec><jats:sec><jats:title>Conclusions<\/jats:title><jats:p>Approximately one\u2010third of all participants had high risk of pressure ulcer development at admission, which led to the application of nursing preventive care. Our study demonstrated that nurses should pay special attention to patients over 50\u00a0years of age, who had significantly lower <jats:styled-content style=\"fixed-case\">B<\/jats:styled-content>raden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale scores. The <jats:styled-content style=\"fixed-case\">B<\/jats:styled-content>raden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale scores significantly increased in the last assessments showing that <jats:styled-content style=\"fixed-case\">B<\/jats:styled-content>raden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale is sensitive to the clinical improvement of the patient. Braden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale correlations with length of stay reveal its importance as predictor of length of stay.<\/jats:p><\/jats:sec><jats:sec><jats:title>Relevance to clinical practice<\/jats:title><jats:p>Nurses should use <jats:styled-content style=\"fixed-case\">B<\/jats:styled-content>raden <jats:styled-content style=\"fixed-case\">S<\/jats:styled-content>cale assessment and consider patients' characteristics and diagnoses to plan more focused preventive interventions and improve nursing care. This study could be the first step to create a preventive protocol based on institutional reality, patients' characteristics, level of risk and affected
