However, traditional forms of support are often inaccessible due to the high costs and complex coordination. Large Language Models (LLMs) show promise in providing human-like dialogues that could emulate social support. Yet, in-depth, in situ investigations of LLMs to support behavior change remain underexplored. We conducted two randomized experiments to assess the impact of LLM agents on user engagement with mindfulness exercises. First, a single-session study, involved 502 crowdworkers; second, a three-week study, included 54 participants. We explored two types of LLM agents: one providing information and another facilitating self-reflection. Both agents enhanced users' intentions to practice mindfulness. However, only the information-providing LLM agent, featuring a friendly persona, significantly improved engagement with the exercises. Our findings suggest that specific LLM agents may bridge the social support gap in digital health interventions.<\/jats:p>","DOI":"10.1145\/3757619","type":"journal-article","created":{"date-parts":[[2025,10,16]],"date-time":"2025-10-16T16:59:10Z","timestamp":1760633950000},"page":"1-44","update-policy":"https:\/\/doi.org\/10.1145\/crossmark-policy","source":"Crossref","is-referenced-by-count":4,"title":["Large Language Model Agents for Improving Engagement with Behavior Change Interventions: Application to Digital Mindfulness"],"prefix":"10.1145","volume":"9","author":[{"ORCID":"https:\/\/orcid.org\/0000-0003-2878-3986","authenticated-orcid":false,"given":"Harsh","family":"Kumar","sequence":"first","affiliation":[{"name":"University of Toronto, Toronto, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0000-0001-6098-9947","authenticated-orcid":false,"given":"Suhyeon","family":"Yoo","sequence":"additional","affiliation":[{"name":"University of Toronto, Toronto, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0000-0002-1228-5774","authenticated-orcid":false,"given":"Angela","family":"Zavaleta Bernuy","sequence":"additional","affiliation":[{"name":"McMaster University, Hamilton, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0000-0002-8755-8984","authenticated-orcid":false,"given":"Jiakai","family":"Shi","sequence":"additional","affiliation":[{"name":"University of Toronto, Toronto, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0009-0000-7292-1560","authenticated-orcid":false,"given":"Huayin","family":"Luo","sequence":"additional","affiliation":[{"name":"University of Toronto, Toronto, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0000-0002-9122-5242","authenticated-orcid":false,"given":"Joseph Jay","family":"Williams","sequence":"additional","affiliation":[{"name":"University of Toronto, Toronto, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0000-0002-5941-4641","authenticated-orcid":false,"given":"Anastasia","family":"Kuzminykh","sequence":"additional","affiliation":[{"name":"University of Toronto, Toronto, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0000-0003-3089-6883","authenticated-orcid":false,"given":"Ashton","family":"Anderson","sequence":"additional","affiliation":[{"name":"University of Toronto, Toronto, ON, Canada"}],"role":[{"role":"author","vocabulary":"crossref"}]},{"ORCID":"https:\/\/orcid.org\/0000-0001-8542-6913","authenticated-orcid":false,"given":"Rachel","family":"Kornfield","sequence":"additional","affiliation":[{"name":"Northwestern University, Chicago, Illinois, USA"}],"role":[{"role":"author","vocabulary":"crossref"}]}],"member":"320","published-online":{"date-parts":[[2025,10,16]]},"reference":[{"key":"e_1_2_1_1_1","doi-asserted-by":"publisher","DOI":"10.2196\/16021"},{"key":"e_1_2_1_2_1","volume-title":"Diogo Almeida, Janko Altenschmidt, Sam Altman, Shyamal Anadkat, et al.","author":"Achiam Josh","year":"2023","unstructured":"Josh Achiam, Steven Adler, Sandhini Agarwal, Lama Ahmad, Ilge Akkaya, Florencia Leoni Aleman, Diogo Almeida, Janko Altenschmidt, Sam Altman, Shyamal Anadkat, et al., 2023.
