Cardiovascular risk (CVR) underlies aging process and longevity. Previous work points to genetic and environmental factors associated with this risk.<jats:italic>Objectives<\/jats:italic>. The aim of this research is to look for any CVR gene-gene and gene-multifactorial\/lifestyle interactions that may impact health and disease and underlie exceptional longevity.<jats:italic>Methods<\/jats:italic>. A case-control study involving 521 both gender individuals, 253 centenarians (<mml:math xmlns:mml=\"http:\/\/www.w3.org\/1998\/Math\/MathML\" id=\"M1\"><mml:mn>100.26<\/mml:mn><mml:mo>\u00b1<\/mml:mo><mml:mn>1.98<\/mml:mn><\/mml:math>years), and 268 controls (<mml:math xmlns:mml=\"http:\/\/www.w3.org\/1998\/Math\/MathML\" id=\"M2\"><mml:mn>67.51<\/mml:mn><mml:mo>\u00b1<\/mml:mo><mml:mn>3.25<\/mml:mn><\/mml:math>years), low (LCR,<mml:math xmlns:mml=\"http:\/\/www.w3.org\/1998\/Math\/MathML\" id=\"M3\"><mml:mi>n<\/mml:mi><mml:mo>=<\/mml:mo><mml:mn>107<\/mml:mn><\/mml:math>) and high (HCR,<mml:math xmlns:mml=\"http:\/\/www.w3.org\/1998\/Math\/MathML\" id=\"M4\"><mml:mi>n<\/mml:mi><mml:mo>=<\/mml:mo><mml:mn>161<\/mml:mn><\/mml:math>) CVR. Hypertension, diabetes, obesity (BMI, kg\u00b7m<jats:sup>-2<\/jats:sup>), and impaired kidney function were defined according to standard criteria. CVR was calculated using Q risk\u00ae. DNA was genotyping (<jats:italic>ACE<\/jats:italic>-rs4646994,<jats:italic>AGT<\/jats:italic>-rs4762,<jats:italic>AGR1<\/jats:italic>-rs5182,<jats:italic>GRK4<\/jats:italic>-rs2960306,<jats:italic>GRK4<\/jats:italic>-rs1024323,<jats:italic>NOS3-rs<\/jats:italic>1799983, and<jats:italic>SLC12A3<\/jats:italic>-rs13306673) through iPlex-MassARRAY\u00ae, read by MALDI-TOF mass spectrometry, and analyzed by EARTDECODE\u00ae.<jats:italic>Results<\/jats:italic>. Antilongevity factors consisted (OR 95% CI,<mml:math xmlns:mml=\"http:\/\/www.w3.org\/1998\/Math\/MathML\" id=\"M5\"><mml:mi>p<\/mml:mi><mml:mo>&lt;<\/mml:mo><mml:mn>0.05<\/mml:mn><\/mml:math>) BMI 1.558 (1.445-1.680), hypertension 2.358 (1.565-3.553), smoking habits 4.528 (2.579-7.949), diabetes 5.553 (2.889-10.675), hypercholesterolemia 1.016 (1.010-1.022), and regular consumption of red meat 22.363 (13.987-35.755). Genetic aspects particularly for HCR individuals<jats:italic>ACE<\/jats:italic>II (OR: 3.96 (1.83-8.56),<mml:math xmlns:mml=\"http:\/\/www.w3.org\/1998\/Math\/MathML\" id=\"M6\"><mml:mi>p<\/mml:mi><mml:mo>&lt;<\/mml:mo><mml:mn>0.0001<\/mml:mn><\/mml:math>) and<jats:italic>NOS3<\/jats:italic>TT (OR: 3.11 (1.70-5.70),<mml:math xmlns:mml=\"http:\/\/www.w3.org\/1998\/Math\/MathML\" id=\"M7\"><mml:mi>p<\/mml:mi><mml:mo>&lt;<\/mml:mo><mml:mn>0.0001<\/mml:mn><\/mml:math>) genotypes were also risk associate. Obesity, smoking, hypercholesterolemia, and frequent consumption of red meat have an additive action to hypertension in the longevity process. There was a synergistic interaction between the endothelial<jats:italic>NOS3<\/jats:italic>genotypes and the severity of arterial hypertension. An epistatic interaction between functional genetic variants of<jats:italic>GRK4<\/jats:italic>and angiotensinogen was also observed.<jats:italic>Conclusions.<\/jats:italic>Cardiovascular risk-related genetic and multifactorial or predominantly lifestyle aspects and its interactions might influence the aging process and contribute to exceptional longevity in Portuguese centenarians.
