{"status":"ok","message-type":"work","message-version":"1.0.0","message":{"indexed":{"date-parts":[[2026,1,21]],"date-time":"2026-01-21T12:57:23Z","timestamp":1769000243596,"version":"3.49.0"},"reference-count":161,"publisher":"Wiley","issue":"4","license":[{"start":{"date-parts":[[2015,2,2]],"date-time":"2015-02-02T00:00:00Z","timestamp":1422835200000},"content-version":"vor","delay-in-days":0,"URL":"http:\/\/onlinelibrary.wiley.com\/termsAndConditions#vor"}],"content-domain":{"domain":["aao-hnsfjournals.onlinelibrary.wiley.com"],"crossmark-restriction":true},"short-container-title":["Otolaryngol.--head neck surg."],"published-print":{"date-parts":[[2015,4]]},"abstract":"<jats:sec><jats:title>Objective<\/jats:title><jats:p>The objective of this study is to systematically review polysomnography data and sleepiness in morbidly obese (body mass index [BMI] \u226540\u00a0kg\/m<jats:sup>2<\/jats:sup>) patients with obstructive sleep apnea (OSA) treated with either a maxillomandibular advancement (MMA) or a tracheostomy and to evaluate the outcomes.<\/jats:p><\/jats:sec><jats:sec><jats:title>Data Sources<\/jats:title><jats:p>MEDLINE, Scopus, Web of Science, and the Cochrane Library.<\/jats:p><\/jats:sec><jats:sec><jats:title>Review Methods<\/jats:title><jats:p>A search was performed from inception through April 8, 2014, in each database.<\/jats:p><\/jats:sec><jats:sec><jats:title>Results<\/jats:title><jats:p>Six maxillomandibular advancement studies (34 patients, age 42.42 \u00b1 9.13 years, mean BMI 44.88 \u00b1 4.28\u00a0kg\/m<jats:sup>2<\/jats:sup>) and 6 tracheostomy studies (14 patients, age 52.21 \u00b1 10.40 years, mean BMI 47.93 \u00b1 7.55\u00a0kg\/m<jats:sup>2<\/jats:sup>) reported individual patient data. The pre\u2010 and post\u2010MMA means \u00b1 SDs for apnea\u2010hypopnea indices were 86.18 \u00b1 33.25\/h and 9.16 \u00b1 7.89\/h (<jats:italic>P<\/jats:italic> &lt;. 00001), and lowest oxygen saturations were 66.58% \u00b1 16.41% and 87.03% \u00b1 5.90% (<jats:italic>P<\/jats:italic> &lt;. 00001), respectively. Sleepiness following MMA decreased in all 5 patients for whom it was reported. The pre\u2010 and posttracheostomy mean \u00b1 SD values for apnea indices were 64.43 \u00b1 41.35\/h and 1.73 \u00b1 2.68\/h (<jats:italic>P<\/jats:italic> =. 0086), oxygen desaturation indices were 69.20 \u00b1 26.10\/h and 41.38 \u00b1 36.28\/h (<jats:italic>P<\/jats:italic> =. 22), and lowest oxygen saturations were 55.17% \u00b1 16.46% and 79.83% \u00b1 4.36% (<jats:italic>P<\/jats:italic> =. 011), respectively. Two studies reported outcomes for Epworth Sleepiness Scale for 5 patients, with mean \u00b1 SD values of 18.80 \u00b1 4.02 before tracheostomy and 2.80 \u00b1 2.77 after tracheostomy (<jats:italic>P<\/jats:italic> =. 0034).<\/jats:p><\/jats:sec><jats:sec><jats:title>Conclusion<\/jats:title><jats:p>Data for MMA and tracheostomy as treatment for morbidly obese, adult OSA patients are significantly limited. We caution surgeons about drawing definitive conclusions from these limited studies; higher level studies are
