This systematic review and meta-analysis aimed to evaluate the effectiveness of early KC (initiated within the first 24\u202fh) vs. conventional neonatal care in very preterm infants (\u226432\u00a0weeks) and\/or very low birth weight (VLBW) infants (&lt;1,500\u202fg).<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec id=\"j_jpm-2025-0202_abs_002\">\n                    <jats:title>Methods<\/jats:title>\n                    <jats:p>\n                      A comprehensive search of MEDLINE, SCOPUS, Web of Science, and CENTRAL databases was conducted. Randomized controlled trials (RCTs) comparing early KC to conventional care in very preterm and\/or VLBW infants were included. Risk of bias was assessed using the\n                      <jats:italic>Cochrane Risk of Bias tool<\/jats:italic>\n                      . Meta-analysis was conducted using R, with relative risks (RR) and 95\u202f% confidence intervals (CI) calculated.\n                    <\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec id=\"j_jpm-2025-0202_abs_003\">\n                    <jats:title>Results<\/jats:title>\n                    <jats:p>Four RCTs, encompassing 1,679 newborns, were included. Meta-analysis revealed a significant reduction in mortality risk (RR=0.81, 95\u202f% CI: 0.67\u20130.98) with early KC. The pooled RR for infection risk was 0.89 (95\u202f% CI: 0.77\u20131.02). The impact on hypothermia was mixed, with moderate heterogeneity observed.<\/jats:p>\n                  <\/jats:sec>\n                  <jats:sec id=\"j_jpm-2025-0202_abs_004\">\n                    <jats:title>Conclusions<\/jats:title>\n                    <jats:p>Early KC may reduce mortality and morbidity in very preterm and VLBW infants although its effects on hypothermia vary by severity.
