The hidden curriculum, arising from role models and clinical practices, plays a key role, particularly in distinct cultural contexts such as Brazil and Portugal.  Objective:  To explore medical graduates\u2019 perceptions of influences on their professional identity during undergraduate training, highlighting similarities and differences between the two countries.  Method:  A qualitative study with 32 students (11 Brazilian, 21 Portuguese), using semi-structured interviews and creative drawing activities was carried out. Data were analyzed through thematic content analysis.  Results:  Three central themes were identified: persistence of the ideal, transformations along the way, and influence of models. Participants maintained the intrinsic motivation to \u201chelp others,\u201d present since entering medical school. Exposure to positive (humanized, ethical) and negative (dehumanized practices) role models shaped their perceptions. Brazilian students emphasized early clinical exposure, while Portuguese students reported shifts in their desired professional settings. The hidden curriculum, mediated by informal interactions, was pivotal in identity formation.  Conclusion:  Professional identity is significantly influenced by role models and the hidden curriculum.
