This paper aims to understand this place-based collaborative governance functioning and how it can be improved for better effectiveness, efficiency, transparency and accountability. Using a qualitative methodology, through the realization of interviews to the responsible of a Portuguese local health care system (Baixo Vouga), we obtained a structured vision from practitioners, focusing on how they: i) decide about the hierarchy of priorities; ii) identify the best practices; and iii) formulate proposals to strengthen place-based health policies. The main findings revealed that four local components influence most the health determinants - environmental attributes; architecture of proximity care network; sociocultural practices; and community representations - and that the current priority for local health units is to maximize the differentiation of supply.
