The paper is based on preliminary findings from an ongoing research and innovation project, called &amp;ldquo;Tele-homecare, chronic patients and the integrated healthcare system&amp;rdquo; (the TELEKAT project) that employs triple interventions related to patients, professionals, and the organization of care.
