In order to make this shift, health care organizations must have the ability to manage entire populations as well as manage care at the individual level. To proactively manage populations and individuals within these populations, organizations must be able to know, identify and predict what will happen within a population, engage patients and providers to take action and manage outcomes to improve health and care.
