Indeed a substantial amount of emergency hospitalization is linked to medication &amp;ndash; 5% to 10% according to some studies. Methods and tools helping all healthcare providers to have a better knowledge of the complete medication plan are therefore required in order to limit side effects linked to an insufficient knowledge of what the patient is taking.
