The study is performed within a framework of a project for development a complex unified medical information system capable to work on large scale (on population-level). The presented experimental studies show that predictive modelling enables unified detection of remote patients' state using self-measuring and distributed electronic health records. In addition, an experimental mobile application is presented as a tool for data collection and user-system interaction during chronic disease monitoring and treatment.<\/jats:p>","DOI":"10.3233\/978-1-61499-975-1-309","type":"book-chapter","created":{"date-parts":[[2025,2,21]],"date-time":"2025-02-21T09:02:47Z","timestamp":1740128567000},"source":"Crossref","is-referenced-by-count":1,"title":["Personalized Assistance for Patients with Chronic Diseases Through Multi-Level Distributed Healthcare Process Assessment"],"prefix":"10.3233","author":[{"family":"Elkhovskaya Liubov","sequence":"additional","affiliation":[],"role":[{"role":"author","vocabulary":"crossref"}]},{"family":"Kabyshev Maxim","sequence":"additional","affiliation":[],"role":[{"role":"author","vocabulary":"crossref"}]},{"family":"Funkner Anastasia","sequence":"additional","affiliation":[],"role":[{"role":"author","vocabulary":"crossref"}]},{"family":"Balakhontceva Marina","sequence":"additional","affiliation":[],"role":[{"role":"author","vocabulary":"crossref"}]},{"family":"Fonin Vladimir","sequence":"additional","affiliation":[],"role":[{"role":"author","vocabulary":"crossref"}]},{"family":"Kovalchuk Sergey","sequence":"additional","affiliation":[],"role":[{"role":"author","vocabulary":"crossref"}]}],"member":"7437","container-title":["Studies in Health Technology and Informatics","pHealth 2019"],"original-title":[],"deposited":{"date-parts":[[2025,2,21]],"date-time":"2025-02-21T09:08:16Z","timestamp":1740128896000},"score":1,"resource":{"primary":{"URL":"https:\/\/www.medra.org\/servlet\/aliasResolver?alias=iospressISBN&isbn=978-1-61499-974-4&spage=309&doi=10.3233\/978-1-61499-975-1-309"}},"subtitle":[],"short-title":[],"issued":{"date-parts":[[2019]]},"references-count":0,"URL":"https:\/\/doi.org\/10.3233\/978-1-61499-975-1-309","relation":{},"ISSN":["0926-9630"],"issn-type":[{"value":"0926-9630","type":"print"}],"subject":[],"published":{"date-parts":[[2019]]}}}
