Communication issues arise between care-providers, and the patient is left to deal with potentially conflicting information. The new electronic patient record (EPR) that will soon be implemented nationally opens new perspectives to improve patient medication management. In this context, we propose an integrated model that could help further empower the patient with better communication about medications and considerations for reconciliation processes.
