The CAREPATH project provides an integrated care platform to assist both healthcare professionals and patients and their informal caregivers in the day-to-day management of care plans for this patient population. This paper introduces an HL7 FHIR-based interoperability approach for exchanging care plan action and goals with the patients and collecting feedback and adherence information from patients.
