REACH implements an electronic Prospective Surveillance Model (PSM) to address this challenge by systematically collecting electronic patient-reported outcomes (ePRO) to screen for cancer-related impairments, deliver personalized self-management resources, and recommend referrals to rehabilitation services. Developed collaboratively by healthcare professionals, patients, and experts, the development of the REACH system prioritizes usability, security, scalability, and sustainability. Key components include the Dynamic Survey Controller for managing ePRO content, the Resource Library Manager for organizing patient resources, and the Results Logic Processor for recommendation of tailored resources.
