Advances in electronic health records enable large-scale (\u201cbig data\u201d) analyses to identify risk factors and prescribing patterns. We aimed to map literature applying big data approaches to understand CNCP prevalence, patient characteristics, and opioid prescription patterns in primary care. We searched five databases and grey literature (2010\u20132022) for observational studies using large cohorts (\u2265 33, 028) of adults with CNCP (\u2265 3 months), at least one opioid prescription, and a primary care setting. Three studies met inclusion criteria, all from U.S. medical claims. Chronic pain prevalence ranged from 3.82% to 10.3%. Patients were predominantly female, with common comorbidities including depression and anxiety. About 30% used opioids, often at high doses.
