{"status":"ok","message-type":"work","message-version":"1.0.0","message":{"indexed":{"date-parts":[[2026,6,19]],"date-time":"2026-06-19T13:58:33Z","timestamp":1781877513899,"version":"3.54.5"},"reference-count":0,"publisher":"IPEDSS - Instituto de Pesquisa e Determinacao Social da Saude Ltda","issue":"02","content-domain":{"domain":[],"crossmark-restriction":false},"short-container-title":["RCI"],"published-print":{"date-parts":[[2026]]},"abstract":"<jats:p>RNA-based therapies represent one of the major innovations in contemporary molecular medicine, as they enable targeted interventions in gene expression, RNA processing, protein synthesis, and antitumor immune responses. This integrative review aimed to analyze advances in RNA-based therapies for the treatment of chronic and oncological diseases. Twenty-five studies published between 2014 and 2025 were analyzed, selected from recognized scientific databases and high-impact international journals, including clinical trials, translational studies, and human clinical investigations. The main platforms identified included RNA interference, small interfering RNAs, antisense oligonucleotides, messenger RNA vaccines, microRNA mimics, and nanotechnology-based molecular delivery systems. The findings demonstrated greater clinical consolidation of RNA-based therapies in chronic, genetic, metabolic, cardiovascular, and neuromuscular diseases, particularly in conditions with well-defined molecular targets, such as transthyretin amyloidosis, hypercholesterolemia, primary hyperoxaluria type 1, acute hepatic porphyria, spinal muscular atrophy, SOD1 mutation-associated amyotrophic lateral sclerosis, and hypertriglyceridemic syndromes. In oncology, substantial translational potential was observed, especially in personalized mRNA vaccines, neoantigen-based immunotherapy, microRNA mimics, and RNAi strategies targeting oncogenes. However, challenges related to molecular delivery, toxicity, tumor heterogeneity, cost, access, and clinical validation still limit broader implementation.
