Koppel in 1960 evoked the after-effects of lesions of the posterior tibial nerve. Keck in 1969 was the first to describe compression of the posterior tibial nerve by the internal annular ligament. It was a young soldier who, after intensive training, had bilateral plantar anesthesia. The opening of the internal annular ligament had allowed a total recovery in 48 hours. While non-operative management has traditionally been favored, advances in surgical techniques have expanded treatment options, including intramedullary nailing and plate fixation. However, the optimal approach remains controversial, especially in paraplegic patients with distal femoral fractures. This case report highlights the challenges and complications associated with surgical management in this unique patient population\n\nCase Report: A 45-year-old male with T2 paraplegia presented with bilateral distal femoral shaft fractures following a motor vehicle accident. He underwent bilateral retrograde intramedullary nailing to preserve mobility and independence. Postoperatively, the patient developed a hypertrophic non-union on the right side with hardware migration, requiring revision with plate and screw fixation. Subsequently, he developed painful hardware on the right and severe heterotopic ossification on the left, significantly impacting his quality of life. Although hardware removal was recommended, the surgeries were never performed due to the patient\u2019s clinical course and eventual death from unrelated causes.\n\nConclusion: Clinicians should be aware of the potential complications associated with intramedullary nailing and revision open reduction internal fixation in paraplegic patients who present with bilateral distal femoral shaft fractures. Case Report: We present a 65-year-old female with no reported medical history, who presented to our institution with left clavicular pain and wound drainage 8 days after she was struck by a motor vehicle in her home country of Guyana. She was found to have a missed open clavicle fracture with an associated severe infection. She was subsequently treated with irrigation, debridement, and distal clavicle excision.  Conclusion: We present this unique case with a potential procedure which could prove beneficial in cases of infection, trauma, or oncologic lesions in which the distal clavicle is deemed unsalvageable. Children often present with comorbidities and concomitant diseases that affect the respiratory and cardiovascular functions. Significant blood loss and prolonged operating times can pose significant risks. Airway management in a child with a syndrome can be both difficult and challenging. Orthopedic tumor surgery may be complicated by chemotherapy treatment, anesthetics can be affected by drug interactions, and postoperative pain management can be complex. In this chapter, we review common coexisting diseases in pediatric patients undergoing orthopedic surgeries. While it predominantly affects adults, pediatric cases are exceptionally rare, often leading to delayed diagnosis due to non-specific early clinical manifestations.\n\nCase Report: We report the case of an 11-year-old boy with progressive gait abnormalities and rigidity, initially misdiagnosed as an orthopedic disorder. The patient exhibited a forward-leaning posture, absent lumbar lordosis, and significant muscle tightness, leading to a primary surgical intervention. Despite initial improvement, symptom recurrence prompted further evaluation, ultimately raising suspicion of SPS. The absence of pyramidal or extrapyramidal signs complicated early diagnosis, highlighting the challenges in recognizing SPS in pediatric patients.\n\nDiscussion: SPS diagnosis relies on clinical criteria, serological markers, and electromyography. Autoantibodies against glutamic acid decarboxylase (anti-GAD) are commonly associated, although other markers may be present in atypical forms. Pediatric SPS is often misdiagnosed as dystonia, metabolic myopathy, or an orthopedic disorder. The underlying pathophysiology involves autoimmune-mediated dysfunction of GABAergic inhibitory neurons, resulting in loss of muscle control. Treatment strategies include immunomodulatory therapies such as intravenous immunoglobulins and rituximab, alongside symptomatic management with baclofen and benzodiazepines. Long-term rehabilitation is essential to prevent complications and improve functional outcomes.\n\nConclusion: This case underscores the diagnostic complexity of pediatric SPS and the importance of multidisciplinary management. Early recognition is critical to prevent unnecessary surgical interventions and optimize therapeutic strategies. Most of these patients required staged debridement of\nthese wounds and implementing negative pressure wound therapy.\nPurpose: Due to the high intensity of modern war hospitals are faced by many challenges - huge numbers of patients with open fractures\nwith soft and bone tissue defects, personnel shortage, long evacuation hours, patients with tourniquet syndrome. The purpose of this study\nwas to point out the effectiveness of the use of negative pressure wound therapy in the treatment of war wounds in the front line hospital.\nMethods: This is a retrospective case series study, which was conducted among war injured patients. Treatment strategy is the usage of\nmultiple debridements in combination with NPWT. The pressure of the VAC systems ranged from 90 to 130 mm Hg, 120 mm Hg being\nthe average. Wound VACs were changed every 3-5 days. Treatment goals were staged wound closure or wound preparation for complex\nreconstructive surgery. During this study we used several criteria to evaluate successful treatment \u2013 WBC, soft tissue conditions, wound\nclosure with no inflammation signs.\nResults\n1. Debridement in combination with NPWT gives great results in staged wound closure.\n2. NPWT eliminates daily dressing changes which allows a large number of doctors to participate in urgent care of the wounded and\nreduces the load on medical staff.\n3. NPWT provides active wound care during evacuation stages.\n4. NPWT is extremely important in tourniquet syndrome management (helps to differentiate between viable\/non-viable tissues).\nConclusions: NPWT is of great importance during war time. The five-year overall survival is reported to be around 40%. Lymph node metastasis, that is categorized into stage IV in the latest American Joint Committee on Cancer (AJCC, 8th edition) staging system, is characteristic in CCS. We here focused on the impact of nodal metastasis on prognosis in CCS.  Methods: We performed a retrospective analysis of 37 patients with CCS who were treated between 1982 and 2023. This study investigated age, gender, tumor size, tumor location, AJCC stage (7th and 8th edition), presence of lymph node or distant metastasis, presence of surgery, chemotherapy, radiotherapy, and surgical margin. Statistical analysis was conducted to evaluate the impact of each factor on survival.  Results: In our cohort, tumor location, tumor size, and stage were significant prognostic factors. Notably, the disease-Specific Survival (DSS) in patients with nodal and\/or distant metastasis was significantly shorter than those with no metastasis while DSS in the patients with nodal metastasis was not significantly different from those with distant metastasis.  Conclusion: Consistent with the latest AJCC 8th edition in which nodal and distant metastasis are both categorized into stage IV, there was no difference in DDS between nodal metastasis and distant metastasis in CCS in the present study. The purpose of this report is to highlight that nail-plate fixation constructs can be performed percutaneously and expeditiously even in extremely old patients; therefore, patient age should not be a limiting factor in choosing this construct to allow for immediate weight-bearing.\n\nCase Report: The patient was a 99-year-old female who presented to the emergency room after a fall. Plain radiographs demonstrated a comminuted supracondylar distal femur fracture with a sagittal intercondylar split (OTA classification 33A3.3). She was indicated for operative repair and was fixed with a combination of a retrograde Stryker T2 alpha nail and Stryker distal femur locking plate. This method was chosen to allow the patient to be weight-bearing as tolerated after surgery so she could immediately start work with physical therapy to work towards getting back to her pre-injury ambulatory status. At 3 months post-operatively, she had minimal pain, no difficulties with activities of daily living, and was ambulating with the assistance of a cane. At 9 months post-operatively, she was ambulating with a cane (pre-injury status). She did not report any pain, and her radiographs illustrated fracture site consolidation. Furthermore, her short musculoskeletal functional assessment score was the same as it was pre-injury (81).\n\nConclusion: This case supports the idea of using a nail-plate combination for repair of intra-articular distal femur fractures, even in the very elderly as the patient\u2019s functional outcome data reached pre-injury levels. Materials &amp; Methods: 40 patients were divided into two groups and were treated with or without ESWT in combination with conventional oral medicine. A visual analog scale (VAS) score of spinal pain, as well as indicators of spinal mobility, Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) and Bath Ankylosing Spondylitis Functional Index (BASFI) scores, inflammatory index (C-reactive protein, blood cell sedimentation rate), and other indicators were compared between the two groups. The Spondyloarthritis Research Consortium of Canada (SPARCC) scoring system was used to evaluate pain and structural damage in the sacroiliac joint. Results: (1) After one month of treatment (T1), VAS, BASDAI, BASFI, and SPARCC scores were lower in both groups than at the start of treatment (T0) (P &lt; 0.05), with greater decreases observed in the treatment group (P &lt; 0.05). (2) Also, at T1, indicators of spinal mobility for the two groups were improved (P &lt; 0.05). (3) ESR and C-reactive protein levels for the two groups decreased significantly at T1 versus T0 (P &lt; 0.05). Conclusion: ESWT combined with oral medication can significantly relieve pain and improve clinical functional symptoms for patients with AS. It can also reduce sacroiliac joint bone marrow edema and control the inflammatory reaction in the sacroiliac joint, which represents a novel, effective, reliable, and safe clinical treatment therapeutic method.
