It also provides a brief comparison of that system with those of Babylonia, Greece and Rome. There is well-recorded evidence of knowledge exchange over many years between Egyptian practitioners on the one hand, and their contemporaries in Babylonia, and later in the Hellenistic and Roman empires.  However, essentially, Egyptian medicine always retained a unique profile, not least in its attitude towards deformity and disability. Finally, this chapter will summarise ways in which ancient Egyptian medicine, treatment, and the relationship between practitioner and patient can be seen to have contributed to modern \u2018Western\u2019 medicine. Modern pharmaceutical treatments are one example. They continue to use, often in synthesised form, fifty percent of the drug sources once present in ancient Egyptian remedies. Finally, perhaps Egypt\u2019s most important medical legacy is the importance of focusing on the needs of the patient: healthcare was accessible at all levels of society; there was an enlightened approach to deformity, disability, and mental illness; and for those deemed unlikely to survive, there was a recognised need for palliative Among those over 80 years of age (octogenarians), this risk becomes markedly pronounced. Our study aimed to assess the predictive ability of four recognized scoring systems for clinical outcomes in octogenarians presenting with NVUGIB. Methods: We compared the Clinical Rock all Score (CRS), Full Rockall Score (FRS), Glasgow Blatchford Score (GBS), and AIMS65 scores for their ability to forecast clinical outcomes. These outcomes encompassed: (1) the requirement for endoscopic intervention, (2) recurrence of bleeding, and (3) 30-day mortality. Results: The study sample comprised 107 octogenarians, including 58 males (54.2%), with a median age of 86 (range 82-89). Ten patients (9.3%) experienced recurrent bleeding, while 19 (17.8%) succumbed within 30 days. The AIMS65 score predicted 30-day mortality with an optimal cutoff value of 2.5, demonstrating 63% sensitivity and 86% specificity. In patients categorized as high-risk by the AIMS65 criteria, a cutoff value of 2 predicted 30-day mortality with 75% sensitivity and 68% specificity. significance have animal connections. In the last edition of this book two species of streptococci were considered of major zoonotic interest, namely <italic>Streptococcus suis<\/italic> and <italic>S. zooepidemicus<\/italic>. Since then, numerous sporadic zoonoses due to other streptococci have been reported, and a newly recognized fish pathogen with zoonotic potential termed <italic>S. iniae<\/italic> has emerged. Changes in nomenclature make the terminology confusing. For example, the organism known as <italic>S. zooepidemicus<\/italic> \u2014 now termed <italic>S. dysgalactiae<\/italic> subsp. <italic>zooepidemicus<\/italic> \u2014 still causes pharyngitis in humans, complicated rarely by glomerulonephritis after ingestion of unpasteurized milk. Pigs remain the primary hosts of <italic>S. suis<\/italic> with human disease mainly affecting those who have contact with pigs or handle pork.<\/p><p>Once a sporadic disease, several major epidemics associated with high mortality have been reported in China. The major change in reports of zoonotic streptococcal infections has been the emergence of severe skin and soft tissue infections, and an increasing prevalence of toxic shock, especially due to <italic>S. suis<\/italic> (Tang <italic>et al.<\/italic> 2006), group C (Keiser 1992) and group G \u03b2 -haemolytic streptococci (Barnham <italic>et al.<\/italic> 2002). Penicillin remains the mainstay of treatment for most infections, although some strains of group C and G streptococci are tolerant (minimum bactericidal concentration difficult or impossible to achieve <italic>in vivo<\/italic>) (Portnoy <italic>et al.<\/italic> 1981; Rolston and LeFrock 1984) and occasionally strains with increased minimum inhibitory concentrations (MIC) for penicillin are reported.<\/p><p>Agents preventing exotoxin formation, such as clindamycin and occasionally human intravenous immunoglobulin, may be used in overwhelming infection where circulating exotoxins need to be neutralized in order to damp down the massive release of cytokines generated by their production (Darenberg <italic>et al.<\/italic> 2003).
